Immunization Information

Health Services requires the submission of health history forms and enforces mandated immunization testing requirements. The University of Connecticut requires a specific immunization form to be completed. These forms will be distributed to students during advising and registration sessions.

All full-time undergraduate students must complete the required immunization form and return this form directly to the Registrar at the Torrington Campus. These forms should be completed and submitted prior to September 1st. The university will temporarily waive this requirement to allow students to enroll for Fall 2008 classes during the summer registration period.

These health forms will be required in order to recieve access to register for subsequent semesters. Failure to submit this information may result in a denial of services.
